The purpose of this form is to notify the Office of Distributed Learning that you are planning to offer a course or program off-campus, online, or by compressed video/cable TV. This information will assist in marketing these courses as well as supporting them for faculty and students, please complete and submit this form online. By collecting information about all such courses, our office will be able to better serve our students, acting as a clearinghouse of distributed learning information.
